Please have the following forms completed and ready for your first appointment.  If you are unable to print and complete the forms for your first visit, kindly arrive 15 minutes early.  If your child has an appointnment for a Physical Therapy evaluation please bring a pair of shorts, sneakers and tank tops.  For feeding evaluations please bring food(s) and utensils that you usually use at home.  Also for all appointments kindly bring any assistive devices (communication devices, walkers, glasses, hearing aides, etc.) and any previous therapy progress reports and physician visit notes.  We look forward to meeting you!
